Athletic Participation

Eureka High School has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.

How to Register

All Students will need to upload their MSHSAA Physical Exam into Final Forms this first year, it will NOT transfer from Privit.

Please register and follow the prompts to create your account, create your students and sign your forms.

All participants MUST use this MSHSAA form for their physical exam and ONLY UPLOAD PAGE 5 of this document to Final Forms digitally.

Registration Deadlines for 2025-2026

Please note, your student(s) CANNOT participate until marked Eligible (highlighted in green) in FinalForms.  The dates below are first dates of the seasons therefore you need to have this information submitted a MINIMUM of 1 week prior to the dates listed.

Fall 2025 Monday, August 11, 2025
Winter 2025-2026 Monday, November 3, 2025
Spring 2026 Monday, March 2, 2026

Please note: the deadline for Water Polo is Monday, February 23, 2026.
